     I was first hired in 1991 at St. Lukes hospital in their Patient Accounts Department.  I was a part of a group called cash posting where I was an Insurance Remittance Processor.  I received a detailed statement from several Insurance companies. I would then create a run sheet in Lotus 123 that would have the patients name, account number and copay amount along with the payment that was being made by the insurance company.  The run sheet would then be sent to the IT department for the information to be uploaded into the patients account to reflect the payment being made.  These insurance payments were sent to the hospital on a weekly basis and had to be processed  within that week.  There were a number of different Insurance companies that we would receive payments on.  A small example below.
